Is a Breast Lump Always Cancer?

If you have just felt a lump in your breast and are searching the internet with a lot of worry, please take a deep breath. No, most breast lumps are not cancer. In fact, especially in younger women, 8 out of 10 lumps turn out to be completely harmless.

Many women in Nepal hesitate to talk about breast lumps due to shyness or fear. Some ignore a lump because it doesn't hurt, while others panic because it does. The truth is, you cannot tell if a lump is safe or serious just by pressing on it. The best thing you can do for your peace of mind—and your health—is to get it checked by a doctor.

Here is a simple guide to what might be causing the lump and what steps to take next.

Common Non-Cancerous (Benign) Lumps

Most of the time, the changes you feel in your breast are natural or harmless. The most common causes include:

  • Breast Cysts: These are simply fluid-filled sacs inside the breast. They often feel smooth and might become larger and tender right before your menstrual period. They are very common and usually harmless.
  • Fibroadenomas: Often called "breast mice" because they move around easily under your fingers. These are solid, rubbery lumps made of normal breast tissue. They are very common in women in their 20s and 30s.
  • Hormonal Changes: Sometimes, your breasts may just feel generally heavy, lumpy, and painful a week before your period starts. This is a normal reaction to hormones.
  • Infections (Mastitis): If you are breastfeeding, a painful, red, and warm lump is usually a blocked milk duct or an infection that can be treated with simple antibiotics.

How to Tell the Difference: Harmless vs. Suspicious Lumps

While you should always let a doctor confirm, there are different patterns that harmless lumps and suspicious lumps tend to follow.

Feature Usually Harmless (Benign) Needs Urgent Checking
Pain Often tender or painful, especially before periods Often completely painless
Movement Slips and moves freely under the skin Feels hard and glued (fixed) to the skin or chest
Texture Feels smooth, rubbery, or like a water balloon Feels hard, stiff, with uneven edges
Other changes Goes away or shrinks after your period Keeps growing; the skin over it looks like an orange peel

A Note for Nepali Patients: A common myth is that "if it doesn't hurt, it is nothing to worry about." This is not true for breast cancer. Early breast cancer usually does not hurt. Never ignore a painless, hard lump.

What Happens When You Visit the Clinic?

Many women delay going to the hospital because they are scared of what the doctor will do. Checking a breast lump at KCC is a simple, standard process called the Triple Assessment. It is designed to give you an accurate answer quickly:

  1. A Gentle Checkup: A breast specialist will ask about your symptoms and gently examine the lump.
  2. A Simple Scan: For women under 35, we usually do a quick, painless Ultrasound (भिडियो एक्सरे). For women over 35, we may also do a Mammogram (a special breast X-ray).
  3. A Small Test (Biopsy): If the scan shows something unusual, the doctor will numb the area and use a thin needle to take a tiny sample of the lump for testing.

You can usually get all these tests done in a single visit, and the results will clear your doubts within a few days.

Quick Questions, Straight Answers

Frequently Asked Questions

Can a breast lump be painful and still not be cancer?

Yes! Actually, pain is more common with harmless conditions like infections, cysts, or period-related swelling. Early breast cancer is usually painless.

Does a lump moving under the skin mean it's not cancer?

A lump that moves freely (like a marble) is usually a harmless fibroadenoma, but you still need a doctor to confirm it with an ultrasound.

Will the doctor remove my breast if I have a lump?

Absolutely not. First, most lumps are not cancer and just need to be monitored. Even if it is cancer, modern surgery at KCC focuses on saving the breast (Breast Conservation Surgery) by only removing the lump whenever possible.

At what age should I start worrying about a lump?

You should not panic, but you should take any new lump seriously, regardless of your age. Even though breast cancer is less common in young women, it can still happen.
